iPad Competition Terms and Conditions

This year, if you sign up to have a new direct debit to pay your rates before 29 September you will go in the draw to win an iPad, kindly donated by the Bank of New Zealand.

Below are the terms and conditions.
  1. By entering the competition or promotion each entrant will be deemed to have accepted these terms and conditions and to have agreed to be bound by them.
  2. The competition opens on the 22 July 2017 when the Rates instalment notices are first mailed out, and closes at 29 September 2017. There is one prize, being an iPad Wi-Fi - 32GB MP2F2X/A to be won.
  3. One winner will be drawn at random on 6 October 2017 from all customers who submit a Direct Debit application, and is received at the council offices before 29 September 2017.
  4. Entries are restricted to new Direct Debit applications; one per person, and that person must be a resident of New Zealand
  5. The prize is not transferrable or redeemable for cash
  6. The prize draw supervisor’s decision is final and no correspondence will be entered into
  7. The winner will be notified by telephone. If the winner cannot be contacted after reasonable attempts to arrange collection, the prize will be forfeited with no right of compensation, and another winner will be drawn. The winner will also be announced on Horizons Regional Council’s social media channels.
  8. Entry is open to New Zealand residents only. Neither Bank of New Zealand employees (as well as their immediate families living at the same address) nor associated agencies of Bank of New Zealand, and Horizons Regional Council employees are eligible to enter into the prize draw.
  9. Failure to supply all requested personal information at the time of submitting an entry might result on the entry being invalid.
  10. No responsibility will be taken for late/lost/misdirected mail, incomplete entries or entries with incorrect postage. Entries will be deemed to be void if stolen, forged, mutilated or tampered with in any way.
  11. By entering the competition winners agree to their names being published and to be photographed and/or interviewed by Horizons Regional Council or The Bank of New Zealand or the promoter and that the promoter may use their names and such photographs and/or interviews for publicity purposes.
  12. All prizes are subject to standard terms and conditions of The Bank of New Zealand who is supplying the prizes.
  13. Horizons Regional Council/The Bank of New Zealand are not liable for damage to prizes sent by post or courier.
  14. Winners will be drawn at Horizons Regional Council Offices, 11 to 15 Victoria Avenue, Palmerston North.
  15. Subject to any applicable law which cannot be excluded, Horizons Regional Council and Bank of New Zealand employees are not responsible for any loss or damage whatsoever that is suffered (including but not limited to indirect or consequential loss) or for personal injury suffered or sustained during the course of accepting or using the prizes.
  16. All entries become the property of Horizons Regional Council and may be used for promotional and marketing purposes, or passed onto a third party involved in the promotion for their marketing purposes.
  17. Under the Privacy Act 1993, all individuals have the right to access and correct their personal information by contacting Horizons Regional Council by writing to Horizons Regional Council, Private Bag 11025, Palmerston North, 4442.

This information is collected in accordance with the Privacy Act 1993 for the purpose of assessing your suitability for employment with Horizons Regional Council. If your application is successful, this form, together with any supporting documentation, will be retained on your personal file. If your application is unsuccessful your application and supporting documentation will be retained for 12 months after which time it will be confidentially destroyed. This form is intended to assess your competence against specific Horizons behaviours and values. Please complete using up to date and relevant examples.

Health Declaration and Criminal Offences Declaration

Please note that your responses to the questions on this form will only be considered to the extent that they affect the position applied for. If you are offered employment with Horizons Regional Council, the offer may be made subject to the satisfactory completion of a medical examination (at the expense of Horizons Regional Council). Any false or misleading information given in relation to your medical history, can result in you being ineligible for work-related injury compensation, including the loss of entitlement for any compensation from ACC, and may lead to the termination of your employment.

Criminal Convictions, Traffic Offences, and Charges Pending

The declaration of a criminal conviction or traffic offence will not necessarily impact on your suitability for employment.  Any convictions and/or offences declared will be considered in the first instance by the People & Capability Department against Horizons Regional Council criteria in line with the job description for the position you are applying for.

You may also be required to undergo a criminal history check through the Department of Courts as part of the recruitment process.  Please note that any false or misleading information given in relation to the questions asked below may result in the termination of your employment.

Criminal Records (Clean Slate) Act 2004

To be protected by the Criminal Records (Clean Slate) Act 2004, you must meet all conditions in Section 7 of the Act (set out as a summary below), before your convictions can be withheld. Convictions not meeting these requirements should be declared on this form.

You must have:

  • No convictions within the last seven years; and
  • Never been sentenced to a custodial sentence, eg. imprisonment, corrective training, borstal; and
  • Never been ordered by a Court following a criminal case to be detained in a hospital due to your medical condition, instead of being sentenced; and
  • Not been convicted of a “specified offence” (eg. sexual offending against children and young people or the mentally impaired); and
  • Paid in full any fine, reparation, or cost ordered by the court in a criminal case; and
  • Never been indefinitely disqualified from driving under section 65 Land Transport Act 1998, or earlier equivalent provision.
